Shoulder pain / South Austin

Shoulder pain treatment in Austin, TX.

Pain reaching overhead, weakness lifting, or a shoulder that has slowly frozen up. Shoulders lose motion quietly, then complain loudly. We restore the motion and rebuild the strength that keeps it working.

The condition

About shoulder pain.

The most common shoulder pattern we see: reduced overhead range of motion from hours of sitting and desk work, then pain when you ask the shoulder to do something overhead. Reaching for a shelf, pressing weight, throwing, or serving in pickleball. The shoulder did not break. It lost motion gradually, and now normal demands exceed what it tolerates.

A quick honesty note: you will hear that bad posture caused this. The research does not show a direct posture-pain link. What the evidence supports is simpler. Shoulders that move through less range get sensitive in the ranges they lost. The fix is restoring overhead motion and building strength through it, not taping your shoulder blades back.

Other patterns need different handling. True rotator cuff weakness shows up as pain or weakness with specific resisted movements. A frozen shoulder is progressive stiffness in every direction that follows its own timeline. Pain right on the bony point on top of the shoulder, tender to press and worse reaching across the body, points to the AC joint. Each gets a different plan, which is why the assessment matters.

Common causes

What is usually behind it.

Lost overhead motion

Desk work shrinks your comfortable reaching range over months. Then overhead demands hurt. Motion restoration is the fix.

Rotator cuff irritation

Pain or weakness with lifting or resisted rotation. The cuff gets overloaded when bigger muscles do not do their share.

Frozen shoulder

Progressive stiffness in all directions. Follows stages over months and needs a plan matched to the stage, not generic exercises.

AC joint pain

Pain on the bony point atop the shoulder, tender to touch, worse reaching across the body. Common after falls onto the shoulder.

Neck referral

Sometimes shoulder pain starts in the neck. If moving your neck changes the shoulder pain, the neck is part of the story.

Training overload

Pressing, throwing, and serving volume that outpaced shoulder capacity. Common in lifters and racquet sport players.

How we treat it

Shoulder pain care at Forever Forward.

Every plan starts with a full one-on-one assessment. No aides, no hand-offs, no 15-minute sessions. We find what is driving the problem, not just where it hurts, then build your plan around it.

Step 1

Test, don't guess

A full hour, one-on-one. Resisted testing separates cuff problems from AC joint from frozen shoulder from neck referral. Different problems, different plans.

Step 2

Restore motion first

Hands-on treatment to regain overhead range, then progressive strengthening through the new range. Motion without strength does not stick.

Step 3

Load for your life

Pressing, throwing, serving, lifting kids overhead. Your plan builds toward your actual demands so the shoulder holds up where you need it.

Questions we hear

Straight answers.

How can I prevent shoulder pain while playing pickleball?

Keep your overhead motion and build serving and smashing tolerance gradually. Most pickleball shoulder pain is an overload story: too many overheads before the shoulder was ready. A preseason of progressive overhead loading prevents most of it.

Do I need surgery for a rotator cuff tear?

Not automatically. Many cuff tears are painless and found incidentally, and many painful ones improve with PT. What matters is your function and goals, not just the MRI picture. We assess honestly and refer when surgery is the right call.

Why does my shoulder hurt more at night?

Lying on the irritated shoulder compresses it, and stillness lets things stiffen. Night pain that consistently wakes you deserves an in-person look to rule out more serious causes.

Can I keep lifting with shoulder pain?

Usually with modifications. We keep you training around the pain while we restore motion and rebuild capacity. Stopping entirely costs strength you will have to rebuild later.
