Sciatica treatment in Austin, TX.
Shooting pain down the leg, numbness or tingling in the foot, aching that sitting makes worse. Sciatica is scary but very treatable. We find the movement pattern that calms the nerve and build from there.
About sciatica.
Sciatica is not a diagnosis. It is a symptom pattern: pain that shoots from the low back or buttock down the leg, often past the knee, sometimes with numbness, tingling, or weakness in the leg or foot. The usual driver is irritation of a lumbar nerve root, often from a disc, and the pattern of your symptoms tells us a lot about what is going on.
The key assessment finding is centralization. Certain repeated movements cause the leg pain to retreat toward the spine. When we find the movement that centralizes your symptoms, we have found your treatment direction. That single finding guides the entire plan and is something only a proper mechanical assessment reveals.
Most sciatica improves with conservative care. The cases that need urgent medical attention are specific: loss of bladder or bowel control, numbness in the saddle area, rapidly worsening weakness like foot drop, or severe trauma. Those are emergency signs. Everything else deserves a thorough PT assessment first.

What is usually behind it.
Disc-related nerve irritation
The most common driver. Leg-dominant pain that changes with specific spinal movements. Centralization testing finds the treatment direction.
Narrowing-type patterns
Aching legs with standing or walking that ease when you sit or lean forward. Common with age and needs confirmation before planning.
Deep gluteal irritation
Buttock-dominant pain that mimics sciatica but behaves differently with testing. The plan differs, so the distinction matters.
Prolonged sitting
Long commutes and desk days sensitize the nerve. Changing positions frequently matters as much as any exercise.
Deconditioning
A spine and hips that lost strength and motion tolerate less before the nerve complains. Rebuilding tolerance is the long game.
Previous episodes
Sciatica that keeps returning usually never got the movement strategy that centralizes it. Finding that direction breaks the cycle.
Sciatica care at Forever Forward.
Every plan starts with a full one-on-one assessment. No aides, no hand-offs, no 15-minute sessions. We find what is driving the problem, not just where it hurts, then build your plan around it.
Find your direction
A full hour, one-on-one. Repeated-movement testing identifies whether your leg pain centralizes and with which direction. That finding drives everything.
Calm the nerve
Positioning strategies, specific exercises in your directional preference, and hands-on care as needed. The goal is leg pain retreating and function returning.
Build resilience
Progressive core, hip, and back strength so the nerve stays calm under real life: sitting, driving, lifting, and your sport.

Straight answers.
Will sciatica go away on its own?
Often it improves, but slowly and sometimes incompletely. The right movement strategy speeds it up dramatically. Waiting months without a plan risks the nerve staying sensitized.
Do I need surgery for sciatica?
Most cases do not. Surgery is for progressive weakness, emergency red flags, or leg-dominant pain that fails good conservative care over time. We assess honestly and refer when it is warranted.
Is walking good for sciatica?
Usually yes. Walking is one of the best things for most sciatica patterns, and pain that eases with walking is a reassuring sign. If walking consistently worsens leg symptoms, get assessed before pushing through.
Why does sitting make it worse?
Sitting loads the lumbar discs and keeps the nerve in one sensitized position. Frequent position changes, standing breaks, and the right directional exercises counteract it.
